14-bit MIDI Implementation

Hey @danngreen - wondering if we could ever get 14-bit MIDI support. Usually having the MSB sent on cc channels 0-31 and the LSB for the corresponding channel from 32-63.

There would be very great utility for this - mainly that any CV to MIDI module now basically adds more ins for the MM. 7 bit resolution is way too low unfortunately, as it easily becomes visibly stepped, and slew limiting can distort the input. This would be huge as Befaco CV thing, VCMC, Droid X7 etc. can all add ins to the MM.

Seems easy implement as a feature you can enable/disable in the preferences.
